This Notice describes how Medea Care LLC (d/b/a Odelle) may use and disclose your Protected Health Information ("PHI"), and how you can access it.
Our Duty
We are required by law to maintain the privacy of your PHI, to provide you with this Notice, and to notify you in the event of a breach compromising your PHI.
Permitted Uses & Disclosures
- Treatment: sharing your PHI with Dr Telx network physicians and the pharmacies coordinating your care.
- Payment: processing charges and refunds related to your services.
- Operations: quality improvement, training, and internal compliance.
- Legal requirements: when required by federal or state law (e.g., public health, court orders).
Any other use or disclosure of your PHI requires your written authorization, which you may revoke at any time.
Your Rights
- Request a copy of your PHI.
- Request corrections to your PHI.
- Request restrictions on certain uses or disclosures.
- Request confidential communications (e.g., email only).
- Obtain an accounting of disclosures of your PHI.
- Receive a paper copy of this Notice.
- File a complaint with us or with the U.S. Department of Health and Human Services (HHS), without retaliation.
